Jeffery Goldstein is a 52-year-old white male who has had human immunodeficiency virus (HIV) for twelve years. He recently had several opportunistic infections and a change where his HIV has manifested into AIDS.
Primary Disability/Difficulties. Mr. Goldstein was asymptomatic for 10 years. Over the last 24 months, his CD4 count has dropped and he has developed a series of opportunistic infections. His symptoms include severe diarrhea, fever with sweats, painful and open rashes, thrush in his mouth, and extreme fatigue. Most recently, he has experienced ascending paralysis which his doctor suspects is related to Guillain-Barre syndrome.

Education. Mr. Goldstein received his undergraduate degree in accounting and his master’s degree in international tax. He has worked his entire career as an accountant. He passed his CPA exam and teaches an evening accounting class at a local college every spring semester.
Work Experience. He works for a local firm as a CPA. He has held this same position for 20 years. He states that he is “good enough to keep but not good enough to promote.”
Home. Mr. Goldstein lives in a house in the city. He is responsible for all the housekeeping duties and states that due to fatigue he is finding it very difficult to manage them to his own standards. He “is independent in every way and has been” since he “was 17 years old.”
Areas of Interest. Mr. Goldstein is an active member of the Gay Men’s Chorus and a local book club. He is considering dropping out of both due to his fatigue.
Transportation. Mr. Goldstein drives and owns his own car. He also lives near public transportation and takes the train as often as possible. He does not have an accessible parking permit but does qualify for one.
Assistive technology: Mr. Goldstein does not currently use any assistive technology. He would like an AT evaluation and specifically asked about technology solutions for tele-working, He saw a news story about tele-presence robots and hopes this type of technology could help him keep his job. He has used Skype to participate in meetings in the past. He feels that the tele-presence robot would allow him and his coworkers to navigate the office and be able to “be there” without having to physically be in the office. He says his office has “an out of sight, out of mind” mentality, and he wants to make sure he stays connected and employed as long as possible.
DESCRIPTION OF THE PRESENTING PROBLEM
Mr. Goldstein came to vocational rehabilitation for assistance in keeping his job, understanding his rights to work, and finding technology to assist him save energy in the work place. Mr. Goldstein wants to tele-work and/or work a modified schedule. He is experiencing a lot of symptoms related to AIDS, especially fatigue, during the day, and he has not disclosed his HIV/AIDS status to his employer. He is “extremely stressed out” about disclosing his HIV/AIDS status. Mr. Goldstein said his “boss is a horrible person,” and he is having “difficulty communicating effectively with him about most everything.” He stated that “there seems to be increased tension” and he is concerned about losing his job.
