Inequalities in Health Care in the United States
The committee was given the duty of reviewing the situation of health inequalities in the United States, as well as investigating the underlying conditions and root reasons that contribute to health inequities, as well as the interrelated nature of the elements that cause them (drawing from existing literature and syntheses on health disparities and health inequities). The committee examines health inequalities in the United States by race and ethnicity, gender, sexual orientation and gender identity, and disability status in this chapter, focusing on populations that are disproportionately affected by unfairness. In addition, this chapter summarizes data on military veterans as well as variances between rural and urban areas. Existing literature, comprehensive reviews (AHRQ, 2016; NCHS, 2016), and current studies were used by the committee. The report includes examples of communities that are taking action to address the core causes of health disparities in Chapters 2 and 3. These brief samples are meant to serve as examples of the work that communities around the country are doing. The paper delves deeper into nine instances of community-driven solutions to enhance health equity in Chapter 5.
HEALTH DISPARITIES
according to NUR 533 Discussion Population Based Health Disparities, ror the purposes of this report, health disparities are inequalities in the attainment of full health potential among certain demographic groups in the United States, as evaluated by differences in illness incidence, prevalence, mortality, burden of disease, and other adverse health conditions (NIH, 2014). While the term disparities is frequently used or construed to refer to inequalities between racial or ethnic groups, disparities can exist in a variety of other aspects, including gender, sexual orientation, age, handicap status, socioeconomic position, and geographic location. All of these elements, in addition to race and ethnicity, impact an individual’s potential to reach maximum health, according to Healthy People 2020. (Healthy People 2020, 2016). Indeed, evidence on health inequalities shows that health outcomes differ between and within all of the aforementioned identification categories. Health inequities—systematic variations in the health of groups and communities in uneven social positions that are avoidable and unjust—can cause health disparities (Graham, 2004). These are the kinds of inequities that the committee’s charge reflects, and which will be addressed throughout the rest of this report. We describe health inequities affecting communities across several dimensions in this section.
