One of the concepts I covered last week was Orem’s Self-care Deficit Theory. Orem’s concept prioritizes patient independence over self-care with the ultimate goal of surpassing human limitations to self-care (Current Nursing, 2020). When a person’s capacity for self-care, health maintenance, and overall well-being is considered, they are better prepared to take responsibility for their own or others’ health (Shah, et. al, 2013). Because this principle can be quite customized from patient to patient or from patient’s family to patient, its application can vary significantly depending on the group being targeted. This approach, for example, could be applied differently to adult and pediatric populations depending on age and developmental stages. This pediatric theory would most likely place a strong emphasis on educating and assisting parents in providing care. The stage of the illness or sickness may also influence how this hypothesis changes. In the pediatric cardiovascular ICU where I work, for example, the child’s defect, arrhythmia, or illness has a significant impact on the patient’s need for self-care. In newborns with untreated congenital cardiac defects, tube feeding or oxygen therapy may be required. In this case, much more instruction would focus on the parent’s ability to use and manage these modern medical devices. I could also be a young patient who was recently treated on the same unit for mechanical valve replacement. This patient may require full or partial care after surgery, but these limitations will change as the patient’s condition improves and he or she regains independence. Even though many of the teenagers in this situation have the physical and mental capacity to manage their own medications, they would be guided and supported. This child may also require anticoagulant therapy instruction.

Ultimately, because deficits can be flexible and dynamic in nature, how Orem’s Self-Care Deficit Theory is applied to a demographic focus varies greatly from scenario to situation.

Orem’s theory is a grand nursing theory that covers a broad range of topics with general concepts that apply to all nursing. Nursing is required when an adult, or the parent or guardian of a child, is unable or unwilling to provide effective self-care. Conditions caused by illness, injury, or disease can cause self-care deviations. There are five ways to help others: acting or doing for others, guiding, supporting, and teaching others, and providing an environment that promotes demands and future development. When nursing care is required, it is clearly stated. Her approach is contemporary in terms of health promotion and maintenance, but the emotional needs are not addressed. This theory promotes independence and indicates that all patients want to care for themselves and recover faster by performing their own care.

These two theories will assist the nurse leader in understanding their role, the role of patients, and how to promote or facilitate health equilibrium. Humans are empowered when they can make decisions and manage their own care. Caring is essential in nursing and must be prioritized, particularly in this age of cost containment and efficiency.
