Transition plans are often put in place to manage the changing HIM system environment and ensure a smooth transition to new or upgraded systems for end users.
This assessment provides an opportunity for you to develop a plan for supporting end users in transitioning to a new or upgraded HIM system.

Requirements
Research best practices for supporting end users in transitioning to a new or upgraded HIM system. Then, using the APA Style Paper Tutorial [DOCX], develop a transition plan containing best practices that address resource needs, training, scheduling, and ongoing support over the life of the system.
