Evaluation Method

Once you have developed/discussed 3 strategies in the Section C1, then for Section C2 you can discuss how you would be able to evaluate each of the 3 strategies.

For example, if you decide to create a new position [Educator/Navigator] . . . how would you be able to determine if that strategy is proving to be of value in promoting best practices?  Would you see a decreased “no show” for follow up care because patients have a better understanding of the necessity of follow-up?  If you created a new budget item to provide for transportation costs for follow up visits for patients who have transportation challenges . . . would you expect to see improved compliance with follow up care?  If you created an education program for Community awareness about management of the disease process . . . would you expect to see greater participation by members of the Community in “screening activities”?

Development of strategies and evaluation methods may send up looking like a “hypothetically speaking” proposition.
