The shortage of registered nurse (RNs), and its impact on patient care as the need for healthcare grows. “According to the American nurse association, two to three million additional patients will enter into the Medicare system each year”. As the need for more registered Nurses continues to grow they are an increase in hospital-associated infections in the intensive care units (ICU). Results are due to nursing overload with the low nurse to patient ratio. Hospitals requiring nurses to have a baccalaureate degree to work in the intensive care units, in which adds to the shortage, but necessary.
With not enough RN’s working in the hospital practically ICU, less time is spent in the patient’s room due to increase in nurse workload. Therefore, pathogens are transferred among patients, due to less quality of nursing care. Based on the evidence provided by the American Association of Colleges of Nurses (AACN) a national survey on consumers’ experience with patient’s safety and quality information was done. “Results from this survey found that 40% of Americans think the quality of healthcare has worsened in the last 5 years”. (Media relations, American Association of College of Nursing). Along with increased workload comes less than effective nursing care. In addition, the quality of work life for Registered Nurses have created a decrease in job satisfaction for Registered Nurses.
According to the American Association of Colleges of Nursing, the need for more RN with a baccalaureate is needed in the Intensive care units, among other healthcare settings. Studies done in ICU on nurses with a two-year degree shows patient mortality and nurse outcomes were shortened by a greater percentage. From these studies, the numbers were lower for a 30-day mortality rate for acute patients. And with the projected RNs shortage hospitals are projected to be understaffed. Nurses are more likely to stay in the nursing profession and advance to a degree with more hospitals offering competitive rates and better incentives.
Along with Registered Nurse shortage results in increase nurse workload, which directly effects nurse performance and increase nurse errors. Nurses will experience increased stress and fatigue on the job. Job satisfaction rate decreases as a result of the low nurse to patient ratio. With nurses required to do mandatory overtime, again introduce a risk of nurse errors and increase stress and fatigue.
Among the Registered Nurse shortage, the nursing field shows not enough nurses with a baccalaureate degree. Studies conducted in the ICU shows less patient mortality rates with nurses having a four-year degree, comes more education and training according to an article, (Journal of the American Medical Association, AASN). Studies also show more patient outcomes with RNs with a Bachelor’s degree compared to a Registered Nurse with a two-year associate degree (ASN), the mortality rate was significantly less.
Hospitals do offer employees tuition reimbursement for 2-year RN degree to advance to a baccalaureate degree. Some hospitals remain extremely competitive with concerns to pay and benefits. Experience nurses make up more than half the working population of nurses and once retired the supply will not be there to meet the demands of nursing care. Reports from the AACN shows the number of Registered Nurses with a two-year nursing degree advancing to a baccalaureate degree. In which Registered Nurses with higher level of education will promote better patient outcomes.
Hospital stay are lessening, which results in more patients in ambulatory care clinic and home care settings. This demand will increase resulting in the need for more Registered Nurses. Reports shows the need for more experienced and skilled nurses will not be met, and will continues to decrease.
Nursing schools are having turn away nursing student applicants again due to the shortage of Registered Nurses. School and training facilities does not have enough Registered Nurses to teach in the classrooms, and in the hospital settings. Nursing schools are continuing to be understaffed and training facilities are not equipped to train the amount of new nurses. This remains an issue of understaffed of Registered Nurse instructors, which ultimately affects patient care.

“According to the 2013 data from the bureau of labor and statistic it is anticipated that by 2020 there will be a need for 1.05 million RN in the US” due to the growing need for healthcare. The nursing shortage projection estimate that Registered Nurse shortage will likely surpass 500,000 by the year 2025.
Once the baby boomers retire the replacements of Registered Nurses will not be met, this is based on projections of nursing shortage and new graduates. Studies show the shortage has impact the effects on patient mortality and nurse outcome. With retaining experienced nurses this would help to lessen the nursing shortage and transfer knowledge to new nurses. This in return would increase patient quality of care and better patient outcomes. New graduates are not given enough training to perform their duties to meet patient satisfaction. With a good mentoring program and in depth orientation for new nurses entering the profession, would aid in the nursing shortage.

With a leadership program in place for new graduates’ nurses job satisfaction and work life would positively impact the nursing profession.
With aging population comes the increase for nurses, and the demand is reported to be unmet. Some areas that are expected to have a greater demand for RNs as the aging population increases is the oncology specialty. Clinical Nurse specialist will be in more demand in all aspects of oncology as ambulatory care and a variety of oncology settings increase.
In conclusion, all hospitals and health care facilities that employ Registered Nurses should stay competitive with rates and incentives. They should offer healthcare and retirement packages to keep Registered Nurses until they retire. More funding needs to be in place and available for nursing school and training facilities to increase the number of Registered Nurses, which will aid in the nursing shortage. All healthcare facilities and hospitals should only require nurses with their four-year degree. This change would steer nursing students to go higher in their studies. Nursing schools should only offer 4 years or higher of an education degree. The result would be the demand for Nurses with a higher degree would stay competitive, consistent and ultimately increase patient outcome.
