Our group chose the article “Crisis Emergencies for Individuals with Severe, Persistent Mental Illness: A Situation-Specific Nursing Theory” by Brennaman (2012). The theory we connected it with was Ball’s mid-range theory. We related this to the Healthy People 2020 topic objectives of treatment expansion and mental health status improvement. According to the Office of Disease Prevention and Health Promotion (2020), the mental health goals aim to improve mental health through prevention and ensuring access to appropriate, quality mental health services in the public population. To summarize our research, Ball’s mid-range theory was applied by stating that explores vulnerabilities that individuals suffering from mental illnesses have, combined with crises experienced by the population. We found that prior research looked primarily on how external factors influence crises. Special skills that should be grasped by nurses are determining whether the client is experiencing an emergency or crisis and the ability to implement the appropriate intervention based on this. Since Ball’s model of crisis states that when people are experiencing crisis, they may exhibit a variety of behaviors stemming from the sense of being overwhelmed and feeling a loss of control, the nurse must be able to identify these early signs and symptoms, and react quickly to treat them.

Another Healthy People 2020 topic that relates to this is access to health services. Since not everyone in the United States is insured, or many people do not seek medical help, it is an important topic to dive into. If an individual is in crisis, and they are not insured or are for some reason scared to access health services, they may not get the treatment they need. According to Healthy People 2020, these individuals are more likely to have poor health status, and less likely to receive medical care. A nursing intervention we would be able to apply to this situation would be to prevent illness in these populations by promoting healthy behaviors for people with psychological issues, this would lead to greater patient trust in the provider, and better patient-provider communication. For the relation of this topic to Ball’s theory, if we as providers were able to identify patients who are more at risk for mental health issues, such as a family history, we would be better able to detect early signs that something may be wrong, and treat earlier than we would have normally. According to Peterson & Bredow (2020), the evaluation of middle range theories tends to be new in nursing. However, it is easy to relate middle range theories such as Ball’s theory to topics we as nurses are subjected to in our daily lives.

My group wrote about maintaining a culture of silence in the neonatal intensive care unit (NICU). Noise can have negative effects on development. For example, noise increases the risks for apnea, oxygen desaturations, as well as leading to fewer calories available for growth. One of the Healthy People 2020 Topics and Objectives is to reduce the prevalence and severity of disorders of hearing and balance; smell and taste; and voice, speech, and language (Healthy People, 2020). In 2020, the goal is to have at least 90.2% of newborns screened for hearing loss before 1 month of age (Healthy People, 2020). According to the Office of Disease Prevention and Health Promotion, 1 to 3 out of every 1,000 children is born with hearing loss. Neonates, particularly premature infants, are at increased for hearing loss due to the constant loud noises that surround them. My group discussed several recommendations to reduce the noise levels on NICUs.

Another Healthy People 2020 objective is to increase public knowledge of how adequate sleep and treatment of sleep disorders improve health, productivity, wellness, quality of life, and safety on roads and in the workplace (Healthy People 2020) Of course babies do not drive or go to work, but sleep is essential for their growth and development. If they are constantly being stimulated, their body cannot perform basic life functions such as cellular metabolism, digestion, and overall growth. According to the Office of Disease Prevention and Health Promotion (2020), adequate sleep is necessary to fight off infections, and performing basic metabolic, endocrine, and neurological functions.

To promote a “culture of silence” in the NICU during non-emergency situations, there are many steps that both parents and caregivers can do. This includes designated “touch-times” for the babies. Every 3-4 hours is appropriate, based on gestational age, and this is to allow babies to get an adequate amount of sleep. Gemin Ribas et. al (2019) studied the effects of Hammock positioning on reducing pain and improving the sleep-wakefulness cycle in preterm infants. By creating this particular position, babies are reminded of the womb. This is the place where babies had their best sleep. By creating a similar intrauterine environment, babies are calmer and have better sleep.
