In 2017, Kartal and Yazici claimed that the usage of information and communication technology has significantly enhanced the standard of care given to patients by nursing personnel. I can honestly say that the digital era has improved my personal effectiveness and efficiency in my line of work. I currently work in a telemetry unit, where the majority of the nursing decisions I make are affected by a combination of my prior knowledge and the information provided by the equipment in our unit. For instance, a patient receiving Betapace loading for atrial fibrillation needs to have laboratory testing and undergo ongoing monitoring during the course of the therapy. When a patient enters our unit, the software my hospital uses immediately delivers me medical directives, including the drug dose and frequency under a certain protocol. Even when a physician’s order is in place, as a professional nurse, my first course of action is to examine the patient utilizing the knowledge and abilities I’ve gained through years of experience. Based on the knowledge they get from using equipment, performing nursing procedures, interacting with others, and having the capacity to analyze obtained data, nurses make clinical decisions (McGonigle & Mastrian, 2018). The vital sign machine, which provides me with data on a patient’s blood pressure, heart rate, oxygen saturation, and temperature, is one piece of medical equipment I employ in this case to further assess the patient. A continuous heart monitor, which enables me to view a more comprehensive picture of the patient’s cardiac rhythm and alerts me if the protocol ordered is appropriate for the patient, is another crucial piece of gear I employ for Betapace loading. In order to avoid the patient suffering a cardiac arrest or torsade de pointes, the procedure also automatically directs me to check the patient’s potassium and magnesium levels before administering the first dose of the medication. When the electrolytes are out of range, the computer launches a task to either substitute the drug with the electrolyte, hold off on taking the medication, or notify the doctor for further guidance. I can create my next steps as a nurse based on the information provided, my knowledge, and my skills, guiding me to practice safely and ensure that patients’ conditions are addressed effectively. Telenursing is becoming increasingly popular with present nursing shortages. Telenursing involves the use of computers, phones, internet, digital monitoring equipment and other technologies to provide nursing care to patients in distant locations. “Telehealth is an important consumer health application because it allows consumers the option to remain in place for care, yet it still faces barriers that include professional licensure, legislative issues, reimbursement, lack of supportive infrastructure, and protection of storage and devices” (Hebda, Hunter & Czar, 2019, p.379). Many consumers are utilizing the option of telehealth nursing because of the convenience it provides. Instead of a trip to the ER, patients are able to receive nursing care for minor issues from the comfort of their home. Telenursing is a reasonable option for people who need immediate medical advice and/or cannot physically get to a doctor’s office right away. However, there are certain limitations to telehealth and telenursing such as the Haight Act of 2008 which states that providers (i.e- doctors, nurse practitioners) cannot prescribe controlled substances to patients they have not physically examined in person. Nurses cannot provide teleheath services to patients outside of the state(s) in which they are licensed. Nurses are still required to adhere to patient privacy policies such as HIPPA laws, which means they must still operate in a professional setting and use fully encrypted web-based systems to transmit patient information (Govtrack, n.d.).
I believe that telenursing has had a positive impact on our career as a nurses because telenursing provides the option to provide care to patients in remote locations who possibly do not have access to medical professionals in their area. One article states “To meet the patients’ needs, and with the additional burden on nursing because of the current nursing shortage, many home care agencies are looking for innovative ways to care for a large number of patients. Telehealth technology offers increased productivity for nurses by decreasing travel time to remote areas, thereby increasing the average daily census. Especially in rural areas, where driving time to patients’ homes can take up the majority of a nurse’s day, new time-saving and patient safety initiatives are imperative, leading to further adoption of telehealth in home care” (Hughes, 2008). This is just another example of the benefits of nursing informatics. As you’ve witnessed in your own practice, such technologies decrease delays in communication, which can save lives in time sensitive situations as with the aforementioned stroke patient.
